Ok, I've done that.  It appears that the hash lookup is failing
periodically,

Oct 15 03:05:37 jonah postfix/smtpd[78064]: match_hostaddr: 80.0.157.230 ~? hash:/usr/local/etc/postfix/mynetworks(0,100)

And yet the mynetworks file hasn't been changed at all:

% ls -l /usr/local/etc/postfix/mynetworks*
-rw-r--r--  1 root  wheel     83 Oct  1 20:56 /usr/local/etc/postfix/mynetworks
-rw-r--r--  1 root  wheel  65536 Oct  1 20:57 /usr/local/etc/postfix/mynetworks.db

There appears to be a bug here. :(
